MUPRC registration Form

The conference is free of charge, but there will be a $20/person fee for those who would like to order lunch. Payments for lunch are electronic, and they are due at the time of registration. Please only one registration form per institution. We will be using this form for our name tags so please list all presenting and non presenting students and faculty members.
